acknowledge that I have voluntarily applied to participate in dog training activities with
OVER THE EDGE AGILITY. I am aware that there are inherent risks and hazards
involved in activities with and around dogs and I am voluntarily participating in these
activities with knowledge of these potential dangers. I am not relying on OVER THE
EDGE AGILITY classes or it’s instructors, trainers and assistants, hereinafter
referred to as “OVER THE EDGE AGILITY” to prevent such occurrences. In order to
participate in OVER THE EDGE AGILITY classes, demonstrations or other activities, I,
being fully informed of such risks and hazards agree to assume all risks of such or
assigns may have against OVER THE EDGE AGILITY, and agree to release OVER THE
EDGE AGILITY from any and all personal injury to myself, my dog, children in my charge,
or harm to property caused directly or indirectly by acts that might occur in OVER THE
EDGE AGILITY dog, classes, events and activities, and further agree to indemnify,
defend, and hold OVER THE EDGE AGILITY, it’s instructors, trainers and assistants,
harmless from damage, loss liability or expenses, including legal costs and attorney’s
fees, which result from any damage caused by myself or dog(s) I own or handle.
I have carefully read this agreement and fully understand the contents thereof. I am
aware that this is a RELEASE OF LIABILITY and contract between myself, and OVER
THE EDGE AGILITY, and sign it on my own free will.
